The Italian striker had several opportunities to score in the first half, as Milan dominated.
Stephan El Shaarawi opened the scoring for Milan after receiving Kevin-Prince Boateng's through ball and flicking it round Samir Handanovic with the outside of his right boot.
Whoops: Mario Balotelli can't make the contact with the ball he desires and Samir Handanovic collects it
Temper temper: Balotelli argues with Inter stalwart Javier Zanetti
How about that? Inter came from behind when Ezequiel Schelotto powered in a header
The goalkeeper could do nothing about the 21st minute strike - but spent the rest of the half denying Balotelli and his team-mates with a string of super saves.
On two occasions the former Manchester City striker miscontrolled or mishit the ball when well placed in front of goal.
The San Siro had more Inter fans than Milan supporters - the former side were at home - and they mocked their former striker.
He was whistled at and there were some reports of Inter fans waving inflatable bananas at him.
Balotelli responded to the taunts by holding a finger to his mouth.
Tears: Schelotto cries as he celebrates with coach Andrea Stramaccioni
Head boy: Schelotto grabs the equaliser with a perfectly placed header
Milan's dominance was largely down to the work of El Shaarawy and Mattia de Sciglio down the left flank.
This was stopped after the break as Javier Zanetti moved across from left back to right back to deal with the danger.
Abbiati, who had a quiet first period, was called in to action as Inter woke up in the second half.
On target: Stephan El Shaarawy opened the scoring with a tidy flicked finish
Derby delight: El Shaarawy celebrates by leaping in to the air
He saved superbly from Freddy Guarin, who could not believe his effort from 10 yards out following Rodrigo Palacio's cross did not result in the equaliser.
But that came soon after when Argentine midfielder Ezequiel Schelotto came on as a 69th minute substitute and two minutes later headed home Yuto Nagatomo's cross.
Inter continued to press after that but could not break through, as Milan fell away.
The draw left Milan third, six points behind second-placed Napoli and 13 points behind Serie A leaders Juventus. Inter are one point behind Milan in fifth.
Leap: Balotelli rises highest and he tries to win the ball in the air
Combative: Tempers flared as both sides grew more aggressive
Atmospheric: Inter fans light flares which filled the San Siro with smoke
Smoked out: Balotelli stands amongst fog caused by the flares being lit
Fightback: Despite Milan's dominance they only scored once and Schelotto made them pay
Out of luck: Balotelli was denied by some great saves but was also culpable of poor misses
